Unlike classic <strong>Gastroesophageal Reflux Disease<\/strong>, most patients may have <strong>no typical heartburn<\/strong>.<br \/>\n<img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-1005\" src=\"https:\/\/www.profdrelifaksoy.com\/en\/wp-content\/uploads\/2026\/05\/b1.jpg\" alt=\"\" width=\"568\" height=\"462\" \/><br \/>\n<strong>Clinical Findings<\/strong><\/p>\n<ul>\n<li>Hoarseness<\/li>\n<li>Need to clear the throat<\/li>\n<li>Globus sensation (feeling of something stuck in the throat)<\/li>\n<li>Chronic cough<\/li>\n<li>Difficulty swallowing<\/li>\n<li>Symptoms that worsen in the morning<\/li>\n<\/ul>\n<p>Since symptoms are generally <strong>non-specific<\/strong>, there is a risk of overdiagnosis.<\/p>\n<h2><strong>Diagnosis of Laryngopharyngeal Reflux<\/strong><\/h2>\n<h3><strong>1- Clinical Scores<\/strong><\/h3>\n<ul>\n<li><strong>Reflux Symptom Index (RSI)<\/strong><\/li>\n<li><strong>Reflux Finding Score (RFS)<\/strong><\/li>\n<\/ul>\n<p>Helpful in clinical practice but <strong>not sufficient for diagnosis alone<\/strong>.<\/p>\n<h3><strong>2- Endoscopic Evaluation<\/strong><\/h3>\n<ul>\n<li>Findings of posterior laryngitis:\n<ul>\n<li>Arytenoid edema<\/li>\n<li>Erythema<\/li>\n<li>Vocal cord edema<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>However, the <strong>specificity of these findings is low<\/strong>.<\/p>\n<h3><strong>3- Gold Standard Test<\/strong><\/h3>\n<ul>\n<li>24-hour <strong>dual-probe pH monitoring<\/strong> or <strong>impedance-pH<\/strong><br \/>\nParticularly recommended in treatment-resistant patients<\/li>\n<\/ul>\n<p><strong>Treatment (evidence-based)<\/strong><\/p>\n<h3><strong>4- Lifestyle Modifications (First-Line \u2013 Strong Recommendation)<\/strong><\/h3>\n<ul>\n<li>Not eating within 3 hours before bedtime<\/li>\n<li>Reducing caffeine, alcohol, chocolate, and acidic foods<\/li>\n<li>Weight control<\/li>\n<li>Elevating the head of the bed<\/li>\n<\/ul>\n<p>The strongest and most sustainable effect lies here<br \/>\n<strong>1- Medical Treatment<\/strong><br \/>\n<strong>2- a) Proton pump inhibitors (PPI):<\/strong> Generally <strong>twice daily, 8\u201312 weeks<\/strong><br \/>\n<strong>2- b) Alginate therapy<\/strong><\/p>\n<ul>\n<li>Forms a reflux barrier<br \/>\nParticularly beneficial in non-acid reflux<\/li>\n<\/ul>\n<p><strong>3- c) H2 receptor blockers<\/strong><br \/>\nLess effective, supportive treatment<br \/>\n<strong>4- Resistant Cases<\/strong><\/p>\n<ul>\n<li>Reassess the diagnosis<\/li>\n<li>Confirmation with pH\/impedance<\/li>\n<li>Alternative diagnoses:\n<ul>\n<li>Functional throat complaints<\/li>\n<li>Muscle tension dysphonia<\/li>\n<li>Allergy \/ postnasal drip<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>5- Surgery<\/strong><\/p>\n<ul>\n<li>Anti-reflux surgery in selected patients<br \/>\nNot routinely recommended for LPR<\/li>\n<\/ul>\n<ul>\n<li>LPR is a <strong>diagnosis that can be overused<\/strong>, laryngoscopic findings are <strong>non-specific<\/strong>, and PPI treatment is <strong>not effective in every patient.<\/strong> A multidisciplinary approach (ENT + gastroenterology) is important<\/li>\n<\/ul>\n<p>LPR is one of the common causes of voice and throat complaints; however, its diagnosis is difficult, lifestyle modifications are fundamental in treatment, and medical therapy benefits selected patients.
